Create an itemised hospital or pharmacy bill

How to create a medical bill PDF
- Select Hospital Invoice or Pharmacy Bill and choose the format.
- Enter the provider details, invoice number, bill date, and patient name.
- For a hospital invoice, add services with quantities and rates. For a pharmacy bill, add medicine items and their purchase details.
- Review line discounts, bill discount, tax, and payments. In hospital mode, check patient-paid and insurance-paid amounts separately.
- Check the preview for the correct bill type, totals, and balance, then use the displayed credits to download the final PDF.
Hospital invoice and pharmacy bill fields
- Provider and invoice: name, address, contact details, invoice number, date, and relevant registration or tax identifier.
- Patient and visit: patient name, with optional patient ID and contact details. Hospital mode includes doctor, consultation, admission, and discharge details.
- Hospital services: service description, category, code, date, quantity, rate, and line discount.
- Pharmacy items: medicine name, code, batch number, expiry, quantity, MRP, rate, and line discount.
- Payments: payment status, method, reference, amount paid, and balance. Hospital mode also includes insurance payment fields.
Medical bill example: charges and payments
As an illustrative hospital invoice, one service at 60 and two services at 20 each give a subtotal of 100. With no discount and tax set to zero for this example, the total remains 100. If the patient has paid 30 and the insurer has paid 50, the recorded payments total 80 and the balance is 20.
These are sample amounts in a single currency, not suggested medical prices or a tax rule. Enter payments actually received and check them against the provider records.
Choose between hospital and pharmacy bills
Hospital Invoice mode is designed around service charges and visit details. Pharmacy Bill mode changes the provider and item fields to pharmacy and medicine details, including batch number and expiry.
The clinic and hospital bill format supports these bill types. Select the appropriate mode before entering details and check the preview after switching.
Review discounts, tax, and medicine quantities
Check each quantity and rate against the actual service or purchase. For medicines, make the description clear about the unit being billed, such as a pack or strip, so the quantity matches the price. Review line discounts and the separate bill discount to avoid counting a reduction twice.
The current form has one bill-level tax percentage. It does not assign different tax rates to individual medicine or service rows. Confirm that the available fields fit the bill you need to record.
Record patient and insurance payments separately
In hospital mode, use Advance / Patient Paid for the patient payment and Insurance Paid for the insurer payment. Optional insurance fields include provider, policy number, and claim number. Check the balance after entering both amounts.
These fields record billing information; the generator does not submit or approve an insurance claim. Read invoice versus receipt when deciding how to describe charges and received payments.
Check the medical bill before sharing
Match the patient name, provider, invoice date, service or medicine descriptions, and payment amounts to the underlying records. Check batch and expiry details in pharmacy mode and visit dates in hospital mode. Include optional personal details only when needed for the bill.

Is the medical bill generator free?
The form and live preview are free to use. The final PDF uses credits, with the cost displayed in the format picker. See pricing for credit packs.
Can I create both hospital and pharmacy bills?
Yes. Select Hospital Invoice or Pharmacy Bill at the top of the form. Each mode shows the relevant provider, item, and payment fields.
Can I include medicine batch numbers and expiry dates?
Yes. Pharmacy mode includes batch number and expiry fields alongside medicine name, quantity, MRP, rate, and discount.
Can I add admission and discharge details?
Yes. Hospital mode includes optional admission and discharge dates, admission number, room type, and bed number in Admission and insurance.
Can I record insurance and patient payments?
Yes. Hospital mode has separate patient-paid and insurance-paid fields, along with optional policy and claim references. The tool records these details but does not process claims.
Can different medicines have different tax rates?
The current form uses a single bill-level tax percentage, not separate tax rates for each medicine. Check whether this matches the invoice you need before downloading.
Can I choose A4 or US Letter for the medical bill?
Yes. Use the page-size control in the preview to select A4 or US Letter. On mobile, open the Preview tab to access the preview controls. Region and currency are available in Bill settings.
Will the final medical bill PDF have a watermark?
The final paid PDF downloads without the preview watermark. Check the patient details, item list, and payment summary in the preview first.
